Preparing For Your Visit

Whether you are new patient or an established patient of our practice, it’s our goal to make you feel as comfortable and as welcome as possible. New patients are encouraged to arrive 30 minutes before their appointments. This will ensure enough time to complete all medical forms and other necessary forms. If you have already checked in online with ZocDoc or the Patient Portal, you may arrive 15 minutes prior to your appointment. Established patients are encouraged to arrive 15 minutes before scheduled appointments.

It is important to bring a government issued ID and your insurance card. Be prepared to provide the following information:

  • Full medical history including hospitalizations, any surgeries
  • A list of all medications including over the counter formularies
  • Medical records from other clinics, hospitals or referring physicians
  • Any X Rays, CT Scans, MRIs or laboratory studies

REFUND POLICY

Occasionally, accounts are overpaid by the insurance and the patient. In the event you believe an overpayment has occurred on your account and you are due a refund, please call 678-679-3300. One of our business office staff will be happy to assist you.

If you believe a refund due to you is a result of a credit or debit card transaction, and the transaction occurred within a 48-hour period, our business office can assist you with a refund to your credit or debit card. If more than 48 hours has elapsed, we can issue a refund check for any overpayment.
